Group safety reporting is dominated by lagging indicators counted inconsistently. A falling incident rate can mean a safer group or a group that has learned reporting is punished.
We report incidents, near misses and leading indicators to one definition everywhere, and include measured conditions where measurement exists — exposure, guarding status, permit compliance, access control. Measured indicators are harder to under-report than self-declared ones.
